Documents & Forms
There are two important documents that you will have to complete when you come in for an exam:
• Patient Information Form: Contains your contact, health insurance, and eyeglasses/ contact lens information, and notice of Privacy Practices.
• Health History: This is a good basis for any healthcare relationship

Here are some do's and don'ts when visiting New Horizons for an eye and/or contact lens examination.
For your (medical) eye exam:
If you already wear prescription glasses, please bring them with you. Also, if possible, please bring with you specific information about any medications you are currently taking. Also bring any recently written eyeglasses prescriptions you may have. The doctor will most probably use dilating drops that will affect your near vision for approximately 2 to 3 hours and will make your eyes sensitive to bright light. It will be best if you do not drive during this time, so please plan accordingly.
For your contact lens exam:
If you have had an eye exam in the past 6 months, bring the record of the exam to your appointment so that we do not have to repeat any of the tests. If you have not had a recent exam, or if you can't get the records from your exam, you'll need to be scheduled for a full exam, as well as a contact lens evaluation. Bring your contacts and glasses to your exam or contact lens fitting, and bring any recently written glasses or contact prescriptions you may have. If you wear soft lenses, wear the contacts to the exam, and bring your glasses. If you wear rigid gas permeable (RGP) lenses, try NOT to wear your contacts for several days prior to your exam or fitting (wear your glasses), then wear your glasses to your appointment, and bring your contacts. The reason for this is that RGPs are hard - they can mold the front of the eye - and this can cause a change in your prescription. If you wear your RGPs just before having your prescription measured, we will find a prescription that works well right after you remove your RGPs, but may not continue to work well if you refrain from wearing your RGPs and allow your corneas (front of eye) to return to their natural shape.
